Dermatits

PATIENT SEEN HAS BEEN PREVIOUSLY DIAGNOSED AS CONTACT DERMATITIS BY DERMATOLOGISTS & FAMILY DOCS.
28 YR OLD FEMALE HAS HAD THE “RASH” FOR SEVERAL YEARS STARTING OUT ON THE SIDE OF THE HEEL ABOUT THE SIZE OF A NICKLE & HAS SPREAD ALONG THE SIDE OF BOTH FEET. EXTREMELY ITCHY AND CYCLIC IN SYMPTOMS. STARTS OUT AS NORMAL SKIN TO RED RASH-HARDENS-GETS SCALY TO CRUSTY & PAINFUL. AT THIS POINT THE SKIN CAN BE PEELED OFF TO REVEAL BABY LIKE PINK NEW SKIN. THE PROCESS THEN REPEATS ITSELF. TIME OF YEAR NO CHANGE.
ALL OBVIOUS ALLERGENS HAVE BEEN ELIMINATED. PATIENT HAS BEEN TREATED WITH EVERY THING FROM ANTIBIOTICS ORALLY & IN CREAMS TO SIMPLE LUBIDERM .
PRESENT MEDS BCP, HBP MEDS & MIGRAINE TREATMENTS. HOWEVER, RASH STARTED PRIOR TO MEDICATIONS.
DOES NOT KNOW WHERE TO GO NEXT… ANOTHER DERMATOLOGIST, PEDIATRIST, OR INFECTIOUS DISEASE.

    Thank you for your question Josh. Without examining the patient, I cannot fully or accurately give a diagnosis but will try to provide some additional suggestions. If the above patient has had a long-standing diagnosis of contact dermatitis and it is not improving this may in fact not be the correct diagnosis. To get a more accurate diagnosis you would want to make sure that topical steroids were also used and make sure that a skin biopsy was taken. If a biopsy was not taken then I’d recommend this as the next step – this can easily be done by a Podiatrist or a Dermatologist. Good luck!
