Dear Jennifer
Usually, a presription is given based on a patient's inital presentation, however, if the patient has been applying topical antifungals and moisterizing creams without improvement, I would recommend a biopsy to be performed. It is a simple procedure and in the end will be able to give the doctor a more accurate diagnosis to the current problem at hand.

Good-luck and thank-you for contacting foot.com,
